This form must be submitted to request a replacement
card for a participant in an Outreach Training program (10-, 16- or 30- hour OSHA course) who completed their Outreach Training through the Region II Atlantic OSHA Training Center (UMDNJ- School
of Public Health, University at Buffalo, and the Universidad Metropolitana). Replacement cards will only be mailed to the trainer.
