Public Health and Evidence-Based Disease Management Programs

Evidence-based disease prevention programs, particularly community-based self-management programs, are an increasingly important component of the health care continuum.  It is critical that public health practitioners be familiar with these programs, the infrastructure for their delivery and how to refer consumers.   This presentation will include an overview of NJ's infrastructure for delivery of evidence-based disease prevention programs, with a focus on the Chronic Disease Self-Management Program, called "Take Control of Your Health.
