Copy of Initial or Most Recent Refresher Certificate Please bring with you a government issued photo ID. We will accept driver's licenses, passports, birth certiificates, state and federal worker ID's, and university and school issued identifications. Please see our "cancellation policy" if you are unable to attend this course. ALL trainees must provide proof of vaccination or negative PCR test from a test taken no more than 72 hours before start of the course (or first in-person day for hybrid courses). This will be verified at the time of sign-in.
